The following immunizations are required for all students entering seventh grade

2 doses MMR (or 2 doses of a measles-containing vaccine and 1 dose each of mumps and rubella vaccines) | |

3 doses of hepatitis B vaccine (or 2 doses if adolescent has received the Recombivx HB 2-dose adolescent schedule option) | |

1 or 2 doses of varicella vaccine (1 dose if less than 13 years of age; 2 doses if 13 or older) or physicial-certified reliable history of chicken pox disease | |
|
1 booster dose of Td or Tdap (if it had been 5 years or more since the last dose of DTaP, DTP, or DT was given) |

While not required, a second dose of varicella vaccine is not recommended for all children under age 13 | |

While not required, annual influenza vaccination is recommended for al school-aged children | |

While meningococcal vaccine is required only for all newly enrolled full-time students attending a secondary school with grades 9-12 (in the case of ungraded classrooms, those with students 13 years or older) who will be living in a dormitory or comparable congregate living arrangement licensed or approved by the secondary school, it is recommended for all 11-12 year olds |
