New Students
Welcome to Saint Agnes School. As required by
Massachusetts General Laws (Chapter 75, sec 15), a child cannot be admitted to
school unless he/she presents a physician's certificate if immunization. A
recent physical exam, as well as pertinent medical history noting any special conditions
or considerations (allergies, hearing/vision problems, etc.) is also
required. Please have a copy of this information from your pediatrician
sent to the school as soon as possible.
All health forms should be completed and received prior to
your child starting school.
Exemptions from the law are allowed for medical or religious
reasons only. Documentation of the exemption by your child's pediatrician
is required before admission to school and then updated yearly.
If your child is transferring from another school, please
request that your child's current school health record be transferred to
Saint Agnes.
