Due at registration:
Kindergarten: Age 5 by September 1st *

Grade One: Age 6 by September 1st *

*Students in grades K and 1 will be screened for readiness before
acceptance. The screening will be done on visitation day or by
appointment.
Grades 2-7 **
1. Registration form and $250 non-refundable registration
fee
2. Copy of baptismal certificate (Grade 2 only)
3. Copy of recent report card
4. Copy of standardized test scores
5. [If applicable] Copy of Special Educational Plan (I.E.P.
or 504)
**Students entering grades 7 and 8 will be given a placement test on
visitation day or by appointment and will be required to meet with the
principal.
Acceptance Procedure
Acceptance is based on space availability and the following criteria:
1. Children currently enrolled in Saint Agnes School
2. Siblings of children currently enrolled
3. Members of Saint Agnes, Saint Eulalia or Saint Camillus
parishes
4. Any other Catholic school
5. All other applicants
All registrations submitted by February 5th will be reviewed
based on the above procedure.
September to January: Early Consideration
To be eligible for early consideration, you would need:
1. A completed registration form
2. $250 nonrefundable deposit (to be
held pending acceptance)
3. A copy of a current report card
and standardized testing (if applicable)
4. A copy of a special educational
plan (IEP or 504 - if applicable)
February to June: Rolling admission based on availability
Confirmation of acceptance will take place in early
February. Please call the school if you have any questions.
Due at acceptance:
1. Release of record form
2. Parent Questionnaire
3. Tuition Payment Preference Form
4. CORI form
5. Grades 6, 7 and 8 - Language choice
6. Health records - According to Massachusetts's law, a
student may not be admitted to school without a health
record
containing a list of required immunizations.
Screening
Process for Kindergarten and First Grade
A screening is required for admittance into Kindergarten and first grade.
After parents submit a registration form, the school will mail a conditional
acceptance letter to the home. Parents are asked to call the school to
make an appointment for a screening. Each screening will take between 10
and 15 minutes and parents may stay with the child.
